1. All pets must be at least 8 weeks of age to travel. The person filling out this form certifies that the pet will be 8 weeks of age at time of travel.
  2. A valid health certificate must be issued within 10 days of travel for domestic travel and 48 hours for international travel. The certification must be given to the flight nanny at the time of pickup of the pet.  Pets over 16 weeks of age will also need a valid rabies vaccine.
  3. A copy of the Emergency Care Authorization form must be completed and must travel with the pet. This form authorizes the pet nanny to get veterinary help for your pet in case of emergency.  The pet nanny will make every effort to contact you before seeking help however, if we cannot reach you it will be up to our discretion to seek care for your pet.  All medical expenses are the responsibility of the booking party.
  4. Please have your breeder supply a small amount of food that will travel with the pet.
  5. A deposit of 50% is due at the time of booking. This amount holds the travel date for you.  If you need to cancel travel then this amount is not refundable.  The deposit will only be refunded if the pet nanny needs to cancel travel.  In that case a full refund will be issued and the pet nanny will make every effort to rebook the travel on a mutually convenient date.
  6. We will keep the person receiving the pet updated through texts and GPS tracking. If we arrive at the agreed upon location and there is no one there to accept the pet every effort will be made to contact the responsible party.  If we are unable to reach the responsible party then a fee of $500 will be charged to offset the cost of rescheduling flights.  The booking party is responsible for this fee.
  7. If the booking party needs to change the agreed upon travel dates/times then it is understood that any fees resulting from this is the booking parties responsibility. If changes result in the cancelation of the travel, then the 50% deposit is forfeited.
