In the event the pet described herein becomes ill or injured and requires veterinary care as the responsible party I, hereby authorize The Wright Air Nanny service to seek medical care from a licensed veterinarian. My signature acknowledges that I have read and accept this document, and I agree to hold The Wright Air Nanny service harmless from any illness or injury of my pet during or after transport.
